At Des Moines Vision Center, we believe in continual improvement of our services. Please take a few moments and share your experience with our office. We appreciate your feedback!
If you currently wear glasses, even on a part time basis, please bring them with you to your appointment. This will allow us to read the prescription and give you a better idea how your vision has changed.
If you currently wear contacts, we ask that you wear them to your appointment. If you would like a new contact lens prescription, please be aware that there is an additional fee not covered by some insurance plans. This contact lens fitting fee is necessary as a contact lens exam is more complex and often requires additional follow up visits. This small fee will cover all contact lens visits needed for one year.
Speed up your visit by completing the necessary form(s) below. For your convenience, you can either fill the form(s) out online or download and print the form(s) and bring it with you to your appointment.

In order to control billing costs, we ask that your portion is paid at the time services are rendered. As a courtesy, we will contact your primary insurance prior to your appointment for a breakdown of your vision benefits. We will bill your primary vison insurance provider. In some cases, we do not bill secondary insurance. We ask that you bring your insurance card with you to your appointment. Please understand that you will ultimately be responsible for all charges incurred in this office regardless of insurance.
